This agreement between
MEEST America Inc., a New Jersey Corporation, having its principal
office Located at 641 Dowd Ave., Elizabeth, New Jersey (hereinafter referred
to as MEEST) and the
Shipper (or here Sender)
Shipper's name
Shall be effective
for one year from the date hereof, and shall continue thereafter from
year to
year unless terminated by either party during the first or any subsequent
year.

The sender agrees to be bound by and abide by the terms, conditions, rules,
procedures
and restrictions as set forth herein
The Shipper agrees
to provide MEEST America Inc. with the following:

Detailed (100%) description of the goods to be shipped. |

Shipper’s full address and telephone number |
| |
3.
Shipper’s government issued ID |
The Shipper acknowledges
that it is prohibited and unlawful to send in a parcel: any
explosives, gasses, flammable liquids, radioactive materials, corrosives,
oxygen rich material,
oxidizers and organic peroxides. Among others include but are not limited
to batteries, lighters,
perfumes, bottled gas, oil based paints, alcohol beverages, drugs and fire
arms, cigarettes.
MEEST America Inc. shall not be liable for the loss of any items that
are prohibited |
from being sent in a
Parcel including but not limiting to cash, precious metals, stones and
jewelry.

to a search of this parcel. |
| If any of the
requirements mentioned above are not met, MEEST |
America Inc. will
not be able to ship Sender’s air parcel.
If any of the prohibited items are found in the air parcel, that package
may be confiscated
and the cost of shipping will not be returned.
